OHS score
Hip pain and function
The Oxford Hip Score (OHS) is a 12-item patient questionnaire developed to assess pain and function, initially in patients undergoing total hip replacement. The recommended modern scoring ranges from 0 to 48, with a higher score corresponding to better functional status.
- Number of questions
- 12 items
- Approximate duration
- 3 to 5 minutes
- Population
- Adults who are candidates for or have undergone total hip replacement
- Modality
- self-administered questionnaire
Terms of contract
Self-administered questionnaire completed by the patient for the affected hip, referring to the last 4 weeks, with only one box checked per question. If one or two answers are missing despite a follow-up prompt, they may be replaced by the average of the other answers; beyond that, the score is not calculated. Keep the 0–48 scoring system and use the same version throughout follow-up. Imputing one or two missing answers using the average only takes place during analysis, after an attempt has been made to complete the questionnaire, and it must be reported. No silent imputation during data entry; more than two missing answers prevents the overall score from being calculated.
Score interpretationOHS
Each question is scored from 0 to 4 and then summed up: total score ranges from 0 (least favorable outcome) to 48 (most favorable outcome). Do not use the old 12–60 system within the same history.

The score is mainly used continuously and longitudinally. Avoid applying rigid categories not intended for the clinical context. Always indicate the direction: higher = better.
